Off-Site Only ISS Services
Under the Texas Medicaid Home and Community-based Services (HCS) and CLASS waivers, enrollees are authorized for a vital habilitative service: Individualized Skills and Socialization (ISS). The state funds up to thirty hours a week of socialization and skill-building in community settings.
However, in rural counties like Bowie, Red River, Titus, and Morris, there are currently zero accessible day facilities operating. The only traditional alternative is staying isolated at home—which leads to cognitive regression and severe caregiver burnout—or facing a grueling 6-hour daily transit to urban day habilitation centers.
Taylor's Gift ISS bridges this rural access gap by delivering 100% Off-Site habilitative services directly inside natural community environments (libraries, parks, markets) chosen by the individuals. To resolve the chronic rural workforce shortage, we formally employ primary Host Home caregivers as part-time off-site attendants. This leverages lifelong relationships to ensure client safety, provides caregiver families with direct administrative compensation, and eliminates transportation barriers.
Guaranteed Staffing & Ratio Compliance
Hiring primary caregivers eliminates rural travel staffing barriers and ensures 100% attendance and ratio compliance (26 TAC §263.2017) during community transits and outings.
Behavioral Safety & Redirection
Caregivers possess deep, lifelong familiarity with the individual's behavioral triggers and de-escalation plans (BSP), preventing public sensory overload crises and programmatic exclusion.
Continuous Habilitation Reinforcement
Socialization skills practiced during community outings (e.g. monetary transactions at markets) are seamlessly reinforced back inside the home, accelerating skill acquisition.
Mitigation of Social Isolation
Replaces grueling 300+ mile daily commutes to urban programs with localized, meaningful civic engagement, libraries, and active volunteerism in the client's home county.
TG-ICHS: Custom Compliance Architecture
