GRACE STEPS – a Texas Off Site ISS

A facilitator may be the person’s guardian, LAR, or host home/companion care provider for an adult with intellectual or other disabilities enrolled in the CLASS, HCS, or TxHmL waiver program. We can employ the facilitator to accompany the individual and support their participation in off-site Individualized Skills and Socialization services and community events.
